Osteopathic physicians excluded because of insufficient osteopathic residency data in the AMA Masterfile. General practitioners excluded because they generally do not undergo full residency training. Referent group: male, attended public medical school, completed residency after 1999, attended non–Title VII-funded medical school and residency program.
