The next few questions are about health problems you might have had at any time in your life.
Have you ever had any of the following ?a,b
    Arthritis or rheumatism
    Chronic back or neck problemsc
    Frequent or severe headachesc
    Any other chronic painc
    A stroke
    A heart attack
Did a doctor or other health professional ever tell you that you had any of the following illnesses?b
    Heart disease
    High blood pressurec
    Asthma
    Any other chronic lung disease such as COPD or emphysema
    Diabetes or high blood sugarc
    An ulcer in your stomach or intestinec
    HIV infection or AIDS

In Nigeria, Lebanon, China and Ukraine the first block of questions (from arthritis to heart attack) was asked only in a 12-month format: ‘ Have you had any of the following in the past 12 months?’, and the follow-up question (below) was not asked.
