Six impairment criteria:
  1. Annoyed or upset you in the last year.
  2. Kept you from doing things or going places with your family.
  3. Kept you from doing things or going places with other children/youths.
  4. Made problems with school work or work.
  5. Caused your teachers or boss to be annoyed or upset with you.
  6. Made you feel bad or upset when the problems were worst.

“Intermediate” rating indicates endorsing “Some of the time” in response to one of six the criteria.“

Severe” rating indicates endorsing “A lot of the time” in response to one of the six criteria.
