A 7-year-old boy presented 18 months following a missed Monteggia lesion. a Lateral radiograph of the right elbow showing an anterior dislocation of the radial head, a small radial head epiphysis, and a long and slender radial neck. b Postoperative radiographs confirm good reduction of the radial head. The osteotomy site was distracted, angulated, and then rigidly fixed with a plate bent to the desired shape. The gap at the osteotomy site was filled with bone graft. c The patient underwent an elbow arthrography at 1 month postoperatively for suspicion of subluxation that was in fact due to a persisting radial head deformity. Note that the radial heads appear much larger and mushroom-shaped compared with the neck, thus perturbing the head–neck ratios. d At last follow-up bone union has been acquired.
