A 7-year-old girl presented 30 months following a missed Monteggia lesion. a Lateral radiographs of the left elbow showing an anterior dislocation of the radial head, a small radial head epiphysis, and a slender radial neck. b Postoperative radiographs confirm good reduction of the radial head. The osteotomy site was distracted, angulated and then rigidly fixed with a plate bent to the desired shape. The gap at the osteotomy site was filled with bone graft. c At 1-year follow-up a lateral radiograph show a nonunion at the osteotomy site, requiring bone grafting with demineralized bone matrix and a new osteosynthesis with a posterior plate according to the tension band principle. d At last follow-up bone union is acquired.
