  1. Cognitive Complaint (usually memory) – Preferably Corroborated by an Informant

  2. Cognitive Impairment (usually memory) for Age and Education

  3. Essentially Normal General Cognitive Function

  4. Largely Preserved Activities of Daily Living

  5. Not Demented
