1 χ2 tests were used to evaluate differences in mean numbers of visits and in proportions of patients who received treated judged to be at least minimally adequate and who received follow-up treatment. The 3 degree of freedom tests were used to evaluate differences across all four sub-samples, while the 1 degree of freedom tests were used to evaluate differences between the severe-moderate and the mild-none sub-samples
