Protocol for Aggressive Hyperglycemia Management

Patient Management
  1. Blood glucose monitoring every 4–6 hours.

  2. Intravenous insulin infusions in patients with admission glucose > 7.8 mmol/L (140 mg/dL), or persistent hyperglycemia (glucose > 7.8 mmol/L on 2 consecutive draws), to achieve blood glucose 4.4 – 7.8 mmol/L (80–140 mg/dL).

  3. Regular insulin administered subcutaneously using a sliding scale to maintain blood glucose 4.4 – 7.8 mmol/L (80–140 mg/dL).

  4. No dextrose-containing fluids

  5. Feeding started as soon as feasible, with a goal to achieve 90% caloric requirement by day 3.
