Note: RADS: Reynolds Adolescent Depression Scale; SIQ-Jr: Suicide Ideation Questionnaire, Jr; PESQ: Personal Experience Screening Questionnaire (alcohol/substance abuse); YSR: Youth Self-Report; CAFAS: Child and Adolescent Functional Assessment Scale.
