Diagnosis and initial treatment of leukemia, CNS malignancy (excludes meningioma and craniopharyngioma), Hodgkin's lymphoma, non-Hodgkin's lymphoma, neuroblastoma, soft tissue sarcoma, kidney cancer, or bone cancer (specific ICD-O codes defining eligible cases within each diagnostic category are provided at www.stjude.org/ccss) Diagnosis date between January 1, 1970, and December 31, 1986 Age younger than 21 years at diagnosis Alive 5 years from the date of diagnosis, regardless of disease or treatment status English or Spanish speaking, because of logistics of questionnaires and interviews Resident of United States or Canada at the time of initial follow up contact
