“Prior admission” occurred within 1 year prior to the index admission. “Demographics” included age, sex, race, payer. “Clinical details” include CCC groups, disposition (non-home, nursing care), number of different major diagnostic groups, number of operative episodes. Model 5 differed from Model 4 by the elimination of 11 variables from the index hospitalization (number of operative procedures) and the prior hospitalizations (prior history of the CCC categories for neurologic, cardiovascular, respiratory, renal, gastroenterological, metabolic, and congenital/genetic conditions, number of operative procedures, and discharge with home health services).
