  1. Introduction - interventionist describes the purpose of the intervention session and what kinds of questions will be asked and discussed

  2. Identity - interventionist assesses the patient's illness representations and the surrogate's representations of the patient's illness by using the following questions: “Tell me about your (your loved one's) kidney disease”, “What does your (your loved one's) kidney disease bring on from day to day?” “How does your (your loved one's) illness affect your (your loved one's) daily living?”
