Three clinical scenarios are indicated in this diagram: (1) spontaneous improvement, (2) improvement in response to therapy, and (3) no spontaneous improvement or response to therapy. Only in scenario 2 is therapy really effective, whereas side effects could worsen outcome in 1 and 3.
