**High-risk features for the primary tumor (T) staging:
Depth/invasion: >2 mm thickness, Clark level ≥IV, Perineural invasion.
Anatomic location: Primary site ear, Primary site non-hair-bearing lip.
Differentiation: Poorly differentiated or undifferentiated.
