Most congenital cardiac malformations
Surgically constructed systemic pulmonary shunts or conduits
Rheumatic and other acquired valvular dysfunction, even after valvular surgery
Hypertrophic cardiomyopathy
Mitral valve prolapse with valvular regurgitation and/or thickened leaflets
