Except for number of mammography centers (FDA) and rural code (USDA), the contextual variables came from the Area Resource File (ARF) of HRSA. Cases from Illinois and Minnesota have been removed.
The population used in the computation is the estimated total number of females in 2003 from the US Census Bureau.
