Family member infected with multidrug-resistant pathogen
Immunosuppressive disease and/or therapy
Entotracheal intubation
High gastric pH
Co-existing cardiac pulmonary or renal insufficiency
Postoperative care (and age >70 years) after abdominal or thoracic surgery
Dependant functional status
