Simplified overview of the algorithm. In addition to the transitions between adjacent therapies as shown by arrows, one can also transition from 0 to 2 or 0 to 3 medications or from 3 or 4 back to 0, 1, or 2. These additional transitions are not shown. The options for therapy involving four agents are generally avoided due to cost, complexity, issues of patient adherence, and difficulty in evaluating the efficacy of each of the agents.
