U.S. estimates are from the 1999–2006 National Health and Nutrition Examination Surveys (NHANES). Except for smoking prevalence (from Argentine National Risk Factor Survey, 2005) Argentina data are from the Buenos Aires sample of the CARMELA study (2003–2004). Data are age standardized to the World Health Organization standard population.[43]
