Target Populations  - Patients with one or more chronic care needs

Equipment  - Telephones, personal computers, interactive voice response  - In-home messaging/monitoring devices  - Biometric devices (e.g., blood pressure cuff, glucose meter)  - Videophones, telemonitors  - Digital cameras

Care Management Strategies  - Disease Management Protocols (DMP)—a set of questions that proactively identify symptoms, behaviors and health factors that affect disease management. They also deliver education regarding the disease and its management.  - Routine monitoring of vital status (e.g., weight, blood pressure, blood sugar)
