Lowest income is based on total household income in Canadian dollars and number of individuals within household: <$15,000 for one to two persons, <$20,000 for three to four persons, and <$30,000 for five or more persons.
Stress index (min. = 0 and max. = 16) is derived by Statistics Canada. A dichotomous variable is created using the median score as cutoff.
Distress index (min. = 0 and max. = 24) is based on the work of Kessler et al. (2002). A dichotomous variable is created using score of 13 as cutoff.
Alcohol dependence is assessed using Composite International Diagnostic Interview Short Form with dependence probability of 0.85 as cutoff.
HS status is based on smoking 20 or more cigarettes per day.
