According to creatinine clearance (Cockcroft-Gault/Shull): normal, > 80 mL/minute; mild dysfunction, ≥ 50–≤ 80 mL/minute; moderate dysfunction, ≥ 30–< 50 mL/minute; severe dysfunction, < 30 mL/minute.
