Summary diagram. The loss of antinociceptive neurons underlies, in part, the hypersensitivity observed in neuropathic pain. Gray triangle indicates the borders of the RVM. We propose that after SNL, antinociceptive RVM neurons (filled circles) retrogradely degenerate after apoptosis of intrinsic spinal cells (squares) and primary afferent fibers. The resulting loss of descending inhibitory tone contributes to cutaneous hypersensitivity.
