  • Modulation of colonization (eg, decontamination)

  • Stress ulcer prophylaxis

  • Transfusions

  • Hyperglycemia

  • Extremes of age

  • Chronic lung disease (especially bronchitis, chronic obstructive pulmonary disease and asthma)

  • Abdominal or thoracic surgery

  • Intubation

  • Duration of mechanical ventilation

  • Immunosuppression

  • Prior antimicrobial use
