The World Bank classifies economies based upon 2008 Gross National Income per capita, which is calculated by the World Bank Atlas Method (http://data.worldbank.org/). The groups are: low-income, $995 or less; lower-middle-income, $996 – $3,945; upper-middle-income, $3,946 – $12,195; and high income, $12,196 or more. The health indicators were obtained from the WHO Global Health Observatory repository of data by country available at http://www.who.int/gho/en/.
