Screen Layout
  • Remove an extra screen for the item”Check if the symptom….”

  • Separate into two screens: “What medications did you take in the last 12 hours?” and “Did you take other medications?”

  • Create a textbox for each medication

Fonts and Punctuations
  • Change the font for the medication for consistency; use capital letters.

  • In the drop down menu for question asking “What time was this medication last taken?”, the minutes selection should be 00–50 in increments of 10.

  • Add a question mark at the end “Do you have any other thoughts or comments?”

  • Change the wording to state “highest pain” instead of “worst pain” and “lowest pain” instead of “least pain” Use bold font for these words.

  • Add space between the question and the line with the visual analog scale.

Appearance
  • Make the body outline diagram larger to occupy screen and eliminate the zoom feature

  • Indicate “front” and “back” on body outline

  • Align the questions consistently

Function
  • When “Submit” is pressed, a screen comes up saying “http://131.179.136.154 the value is:4:”; this needs to be removed.

  • Add a “Clear” button on some screens

  • When “Other” is checked, add “drop down” menu to allow selection of medication names.

  • Add the items for “select words that describe your thought/feelings over the past 12 hours” and the item on “select the words that describe your pain.” In the item “Have been to Sickle Cell Clinic, Hospital, ED, etc in the past 12 hours… If “None of the above” is checked, a free text box appears; this should be removed.
