The pathological classification of osteonecrosis

Grade Histological changes
0 Normal histology
1 Proliferation of the fat cells and bone marrow oedema
2 Bone marrow necrosis without trabecular necrosis, cytolysis, karyorrhexis, karyolysis in the bone cells and bone marrow
3 Additional to the changes in grade 2, subchondral bone death, resorption and new / live bone formation surrounding the dead trabeculae
