*Four-level classification = diagnostic, average risk screening, surveillance, and high risk. The number of prior ICD codes is defined as the total count of prior ICD-9CM codes present from the list of codes in the second column of Table 2. Algorithm developed in training set of 7,515 colonoscopies, and validated in test set of 7,329 colonoscopies.
