HRs and 95% CIs were calculated by using multivariate Cox proportional hazard regression. Model 1 was adjusted for age (y) and sex. Model 2 was adjusted as for model 1 with additional adjustment for enrollment site (4 sites), race (white, nonwhite), education (<high school, high school, college), smoking (never, former, current), leisure-time physical activity (kcal/wk), BMI (kg/m2), waist circumference (cm), and alcohol consumption (g/d).
