Advantages and Disadvantages of surgeries for early stage lung cancer

Surgery Advantages Disadvantages
1) Lobectomy
  • Lowest recurrence rate

  • Entire tumor and lymph nodes removed

  • Greater loss of lung function

  • More extensive and lengthier procedure

2) Segmentectomy
  • Preservation of lung function

  • Low recurrence rate

  • Better tolerated in patients with comorbid medical conditions

  • Greater number of lymph node dissected than wedge resection

  • Better surgical margin compared to wedge resection

  • Technically challenging

  • Recurrence rate not clearly equal to lobectomy

3) Wedge Resection
  • Preservation of lung function

  • Better tolerated in patients with comorbid medical conditions

  • Shorter operative time

  • Highest recurrence rate

  • Lung tissue removed with disregard to anatomy

  • Surgical margins may be less sufficient

  • Cannot remove intrapulmonary (N1) lymph nodes

  • Mediastinal lymph node dissection often limited or not performed
