Diabetes Mellitus (DM): ICD-9 coden (A-code): 250 (A181) and times ≥ 3.
Hypertension: ICD-9 coden (A-code): 401–405 (A260, A269) and times ≥ 3.
Hyperlipidemia: ICD-9 coden (A-code): 272 (A182) and times ≥ 3.
