Eligibility criteria.

Inclusion criteria include age 21+ years and at least one of (A), (B), (C), or (D).
  1. Serial ECG changes

    1. ST-segment changes

    2. New left or right branch bundle block changes

    3. T-wave inversion

    4. New Q waves consistent with ACS

  2. Elevation of cardiac biomarkers (above a level established by the individual hospital as a determination of cardiac myonecrosis)

    1. CK-MB

    2. Troponin

  3. Cardiac catheterization revealing greater than 70% stenosis in a coronary artery

  4. Admission for PCI/CABG classified as urgent, emergent, salvage, or rescue and symptoms of acute ischemia in the 72 hours prior to admission.

Exclusion criteria include:
  1. ACS secondary to aortic dissection

  2. ACS secondary to demand ischemia

  3. Admission from hospice or for palliative care, or under comfort measures only

  4. Admission for trauma

  5. Diagnosis of dementia

  6. Elective cardiac catheterization procedure (PCI/CABG without symptoms immediately preceding procedure)

  7. Perioperative ACS related to surgery

  8. Under custody of a prison system (federal or state)

  9. Pregnant

  10. Patient transferred from another hospital with a length of stay greater than 24 hours at the referring hospital.
