New left or right branch bundle block changes
T-wave inversion
New Q waves consistent with ACS

Elevation of cardiac biomarkers (above a level established by the individual hospital as a determination of cardiac myonecrosis)
CK-MB
Troponin
Cardiac catheterization revealing greater than 70% stenosis in a coronary artery
Admission for PCI/CABG classified as urgent, emergent, salvage, or rescue and symptoms of acute ischemia in the 72 hours prior to admission.
