□ Cardiac problems □ 0 □ 1 □ 2 □ 3 An irregular heart rate often called atrial fibrillation or ventricular fibrillation. A heart attack occurs when blood supply to the heart muscle is blocked, and the muscle, or a part of it, dies. Heart failure occurs when the heart is not pumping effectively and fluid accumulates in the lungs
