Stage	Relevance to transition care
Precontemplation	Introduce the concept and time in which transition will occur. Understand that the patient may develop negative feelings toward the process, including abandonment.
Contemplation	Explain the plan for transition and education goals for the patient.
Preparation	Institute transition care plan.
Action	Complete transition care education.
Maintenance	Self-efficacy.
