Images of a 9 cm hepatocellular carcinoma (HCC) mass in a 76-year-old man
with hepatitis C-related cirrhosis. Transverse CT scans obtained during the
(a) precontrast, (b, c) late arterial, (d) portal venous, and (e) equilibrium
phases demonstrate a hypervascular HCC in the right posterior segment of the
liver. The lesion shows prominent hypoattenuation in the precontrast phase
with a central necrotic portion and relatively early washout during the portal
venous phase. Note the intratumoral heterogeneous enhancement (b, c) and disproportionally
enlarged vessels (arrows in b and c) in the tumor, thus indicating focal aneurysmal
dilatation. Pathology review of the explanted liver revealed a poorly differentiated
HCC.
