Three sample entries in the Assessment_Instrument table. The column names are adopted from the Dialogix schema in Figure 1, with the addition of ParentLOINC which links this table to the main LOINC table, and InstrumentFK which refers to the LOINC PanelElement that codifies the ordering of questions within an instrument. Note that UniqueName must be unique only within the context of a single instrument, thus ensuring that formulas, like that in Example 3, reference the proper variables. Examples 1 and 2 show two different ways of assessing Suspiciousness; and Example 3 shows how formulas are represented using this schema. A Relevance of 1 means that the question is always asked. The ActionTypes ‘q’ and ‘e’ indicate that a question should be asked, and a formula should be evaluated, respectively. None of these items required validation criteria, so that field is blank.
