P-value for association of low-low vs low-high and high-low vs high-high, respectively with ordinal outcome in a partial proportional odds model. P-value for association of all 4 groups with ordinal outcome in an adjusted partial proportional odds model is <0.001.

Discharge characteristics included in the partial proportional odds models were depression symptoms, ADL and IADL dependencies, intervention group, and one-year post-hospitalization mortality risk index. This index uses the following point scoring system: male sex-1 point, 1–4 ADL dependencies-2 points, 5 ADL dependencies-5 points; presence of congestive heart failure-2 points, cancer-3 points, metastatic cancer-8points; creatinine >3.0 mg/dl-2 points; albumin between 3.0–3.4 g/dl-1 point and <3.0 g/dl-2 points.
