The preschool and school age modules of the child health system were not added until 1988 and 1991 respectively, so the totals reflect those children who should have had each assessment by the time of the survey and whose age meant that they should have a record of that assessment on the child health system. 

The percentage of children who had no data recorded on the child health system or who were not registered on the system varied between 38% and 57% for these assessments.
