Cervical spondylotic myelopathy in a 76-year-old man with instability (case 12). Preoperative radiographs showed percentages of slip of 18%, 0%, and 0% in flexion, and the neutral and extension positions, respectively; the slip angles were -11°, -1°, and 6°, respectively at the C4/5 level (A-D). Preoperative magnetic resonance imaging indicated spinal cord compression and signal change at the C4/5 level (E).
