Diagram for evaluation of genotype testing at first-line antiretroviral therapy (ART) failure. Two strategies for management of confirmed virologic failure after first-line ART failure are compared. The genotype strategy represents the implementation of drug resistance genotype testing at the time of first-line ART failure. In this strategy, the switch to protease inhibitor–based second-line ART is dependent on whether the test result indicates the presence of wild-type or resistant virus. The percentages reflect the proportion of the cohort with virologic suppression to <400 copies/mL at 24 weeks, or ART efficacy, on the given regimen.
