Stahl–Lichtman classification of Kienböck’s disease

Stage I—lunate normal on plain film, MRI positive
Stage II—lunate sclerosis on plain film, no collapse
Stage III—lunate collapse with (a) normal proximal row alignment/carpal stability or (b) abnormal proximal row alignment/carpal instability
Stage IV—degenerative changes in the radiocarpal and/or midcarpal joint, fixed scaphoid rotation
