All ORs were adjusted for PSI risk class and additional baseline patient, provider, and site characteristics shown in Table 1. Adjusted ORs were calculated with multilevel logistic regression models that accounted for the clustering of patients within providers and sites; for length of stay, these multilevel models were calculated using discrete proportional hazards models with patients who died during the index hospitalization censored on the day of death.
