The unit of analysis for the hospital data is a hospital-year, and for the county-level data it is a county-year. The counties “in sample” include all counties in New York and California, as well as Cook County, IL (Chicago), King County, WA (Seattle), Harris County, TX (Houston) and all counties in the St. Louis, MO metropolitan area. The “all counties” data include all counties in the U.S. for which data are available. For confidentiality reasons, starting in1989 all mortality data for counties below 100,000 individuals are collapsed into a single unit by state. All variables cover the time period from 1985 to 2003. All dollar amounts are reported in thousands of year 2000 dollars, adjusted by the Consumer Price Index (series CUUR0000SA0).
