Expected trajectory of a patient for whom end-of-life care should be considered. The patient has a limited life expectancy due to age and ICU admission may be a major trigger for loss of independence and subsequent nursing home admission and progressive and more rapid decline. These possibilities need to be clearly and honestly discussed with patient and family as well as the primary clinician to develop a crisis response plan that is both patient-centered and realistic.
