Proposed classification of acute papilledema

Mild papilledema 1–3 quadrants of abnormal elevation of the disc with blurring of the margin, no disc hemorrhages
Moderate papilledema Any of the following:
  • Abnormal elevation of the disc with blurring of the margin in four quadrants

  • Any abnormal elevation of the disc with:
    • Disc hemorrhages (less than or equal to 2)
    • Dilated pre-papillary capillaries
Severe papilledema Any of the following
  • Marked elevation of the disc with blurred margin and loss of the optic disc cup.

  • Any elevation of the disc with:
    • Three or more disc hemorrhages (In the case of doubt graders should categorize lesions as present if they are ≥ 50% certain of their presence).
