For the cell counts for functional impairment, we have provided the denominators because there were some missing data for this variable. Level of mental health and physical health were measured with the Mental and Physical Component Summary scores of the Medical Outcome Study (MOS) 12-item Short Form Health Survey (SF-12); higher scores are more favorable. Functional impairment (measured with the Sheehan Disability Scale [SDS] as described in the Method) and missed work were assessed for the preceding month.
