Level of mental health and physical health were measured with the Mental and Physical Component Summary scores of the Medical Outcome Study (MOS) 12-item Short Form Health Survey (SF-12); higher scores are more favorable. Functional impairment (measured with the Sheehan Disability Scale [SDS] as described in the Method) and missed work were assessed for the preceding month.
