Composite vulnerability scores were calculated by adding 1 for each of the following vulnerability indices met: Education: < 83% of the facility population completed high school; Race/ethnicity: > 30% of the facility population are minorities; Rural/urban residence: average percentage of rural residences among the facility population is > 52%; and Income: average median household income among the facility population is < $45,000.

For diagnostic mammography taken for ‘Additional evaluation of a recent abnormal screening result’ time since previous mammography is measured from the screen which led to the diagnostic mammography rather than the diagnostic mammography itself. For diagnostic mammography taken for ‘Evaluation of a breast problem’, time since previous mammography is measured from the diagnostic mammography.

Include a final recommendation for short interval follow-up, other work-up, additional imaging, clinical follow-up, or routine care

Included a final recommendation for biopsy, FNA, or surgical consult.
