JN is a 67-year-old female patient admitted to the general surgery unit. She weighs 67 kg and was on the following medications PTA: ASA 81 mg PO daily, atorvastatin 20 mg PO daily, ramipril 10 mg PO daily, metformin 1 g PO bid, glyburide 10 mg PO bid, multivitamin daily. HbA1C is unavailable.
How would you dose insulin in this patient while NPO?
  1. 10 units of NPH at hs

  2. 3 units of regular at meal time

  3. a and b

  4. discontinue oral agents, start correction insulin, low scale

  5. no insulin as patient is NPO
