SUD/MDD: Presence of a substance use disorder or major depressive disorder; BSI-12: Brief Symptom Inventory-12 (Derogatis & Savitz, 2000), anxiety and somatization subscales, SDS: Sheehan Disability Scale (Sheehan et al., 1996); PCS: 12-item short form Heath Survey version 2 (Ware et al., 2002); EQ5D-pain: pain item of the EQ-5D (EuroQol, 1990); MOS-SSS (MOS-Social Support Scale; Sherbourne & Stewart, 1991).
