Coordination of Care Among Primary Care Physicians and Hospitalists During the Transitional Care Process

For PCPs When making referrals, list the pertinent issues and the specific questions being asked in referral. Quickly send relevant information to those caring for the hospitalized patient. For Hospitalists Notify PCPs when patients are admitted so PCP can provide needed information. Provide PCPs with progress reports, notification of discharge, and prompt discharge summaries.
